Southern Tier Connect (STC) is a local Care Coordination Organization (CCO) serving 14 counties in New York State, proudly associated with the Office for People with Developmental Disabilities (OPWDD). Our agency assists adults and children with intellectual and developmental disabilities by referring them to appropriate healthcare providers, social programs, and community-based organizations based on the individualized choices of our members.
